For EtG, supplementary tests, and/or screenings exceeding 10 panels, it is advisable to collect 150mg of the sample. The specimen should be weighed on a jeweler’s scale. If head hair is lacking, an equivalent amount of body hair can be collected. In our terminology, head hair pertains only to scalp hair. Body hair includes all other hair types (facial, axillary, etc.).

Process Overview

There are four primary stages in the lab processing of a drug test: Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the initial entry of a sample into the laboratory system. This step includes verifying the integrity of the sample's seal and shipment, assigning a random LAN (Laboratory Accessioning Number), and completing any additional data entry absent from an electronic chain of custody system.

Screening provides an initial broad analysis for drugs of abuse. This cost-efficient method helps eliminate the majority of non-users, but a positive screening needs a follow-up confirmation for court admissibility. Samples with a presumptive positive from Screening move on to confirmatory testing.

For any presumptive positive in Screening, additional hair is extracted from the original sample and prepared for Extraction, where drugs are isolated from hair at a lower concentration compared to other methods (ex. urine or oral fluid), highlighting the complexity of hair drug screening.

Confirmation of a positive result from Screening is done using GC/MS, GC/MS/MS or LC/MS/MS. All presumptive positive samples are washed before confirmation if needed. The process, from Accessioning to Confirmation, is reviewed under CAP (College of American Pathologists) Hair regulations and accredited to ISO / IEC 17025 standards.
