What it is, How it works

Hair holds a historical record of alcohol and drugs by capturing biomarkers within the fibers of the developing hair strand. When gathered near the scalp, hair can offer up to around a 3-month timeframe for detecting alcohol and other drugs. Collecting hair is uncomplicated, relatively hard to tamper with, and straightforward to dispatch.

A sample measuring 1.5 inches, consisting of approximately 200 hair strands (about the size of a #2 pencil) from the closest point to the scalp, will provide 100mg of hair, which is the preferred quantity for analysis and verification. For EtG, extras, and/or analyses exceeding 10 panels, 150mg of the sample is advisable. We suggest weighing the sample on a jeweler's scale. If scalp hair is not accessible, an equivalent amount of body hair can be gathered. When mentioning head hair, it solely refers to scalp hair. Body hair encompasses all other types (facial, axillary, etc.).

Process Overview

The four primary phases involved in the lab processing of a drug test result include Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial entry of a sample into a lab's framework. This includes ensuring that the sample was sealed and dispatched correctly, assigning an arbitrary LAN (Laboratory Accessioning Number), and completing any extra data input absent from an electronic custody chain system.

Screening consists of a preliminary check for drugs of abuse. While Screening serves as a cost-efficient method to exclude drug consumption on most samples, a positive screen requires confirmation to be permissible in court. Any samples that are preliminarily positive in Screening necessitate a subsequent confirmation.

If a sample shows a preliminary positive in the Screening phase, additional hair is extracted from the initial specimen and readied for Extraction. In this phase, drugs are isolated from hair at notably lower concentrations compared to other techniques (e.g., urine or oral fluid), which is why hair drug screening is the most complicated method to execute.

Confirmation of any positive screening outcome is performed through GC/MS, GC/MS/MS or LC/MS/MS. All samples showing preliminary positives are cleaned prior to confirmation as necessary. The entire lab process from Accessioning to Confirmation undergoes review under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
