A sample measuring 1.5 inches with approximately 200 hair strands (resembling the thickness of a #2 pencil) situated closest to the scalp delivers 100mg of hair, the optimal quantity for initial screening and further verification. For EtG, additional analyses, and/or tests exceeding 10 panels, a quantity of 150mg of hair is preferred. Measurement of the sample is advised using a jeweler’s scale. If scalp hair is not an option, a comparable portion of body hair can be collected. By referring to head hair, we specifically mean scalp hair. Conversely, body hair refers to any other hair type (such as facial or axillary hair).

Process Summary

The lab handling a drug test result involves four stages: Accessioning, Screening, Extraction, and Verification.

Accessioning encompasses the preliminary processing of a sample within a laboratory's system. This includes ensuring the sample was correctly sealed and transported, assigning a unique LAN (Laboratory Accessioning Number), and completing data entry supplementary to an electronic chain of custody system.

Screening executes an initial rapid assessment for substances of concern. Although Screening is cost-effective to rule out drug consumption for most samples, a positive screen necessitates confirmation to be legally valid. Those samples preliminarily positive in Screening require secondary verification.

In instances where a sample preliminarily tests positive during Screening, additional hair is extracted from the initial sample and prepped for Extraction. Within this step, drugs are extracted from hair at significantly lower concentrations compared to other methods (e.g., urine or oral fluid), highlighting the complexity of hair drug analysis.

Verification of any positive screening result is conducted using GC/MS, GC/MS/MS, or LC/MS/MS. Prior to verification, all initial positive samples are cleansed as required. The full laboratory process spanning Accessioning to Confirmation is audited under the CAP (College of American Pathologists) Hair protocols and the ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field.
