What it is, How it works

Hair is straightforward to gather, relatively tough to tamper with, and convenient to send.

A 1.5-inch segment containing around 200 hair strands (approximately a #2 pencil diameter) nearest to the scalp will provide 100mg of hair, which is the perfect sample for analysis. For EtG, add-ons, and/or examinations beyond 10 panels, 150mg of the specimen is advised. It is advisable to weigh the specimen using a jeweler's scale. If scalp hair is not present, a matching amount of body hair may be taken. When mentioning head hair, we refer to scalp hair only. Body hair refers to other hair types (facial, axillary, etc.).

Process Overview

The primary stages in laboratory processing of a drug test result include Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial integration of a sample into a laboratory’s setup. This encompasses verifying proper sealing and shipment of the sample, assigning a unique LAN (Laboratory Accessioning Number), and completing any supplementary data input not included by an electronic chain of custody system.

Screening involves a swift preliminary examination for drugs of abuse. Although Screening is an economical method to rule out drug usage on most samples, a positive result requires confirmation to be legally valid. Any samples that test positive during Screening need further verification.

If a sample tests positive during the Screening stage, additional hair is extracted from the original specimen and suited for Extraction. At this stage, drugs are extracted from hair in much lower concentrations than other techniques (e.g., urine or oral fluid), rendering hair drug screening the most challenging technique to execute.

Confirmation of any positive screening result is executed via GC/MS, GC/MS/MS or LC/MS/MS. All potential positive samples undergo washing before confirmation as required. The complete laboratory process from Accessioning to Confirmation is scrutinized under both CAP (College of American Pathologists) Hair designation and accredited to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
