A 1.5-inch snippet consisting of roughly 200 hair strands (comparable to a #2 pencil's size) nearest to the scalp will yield 100mg of hair, which is the ideal quantity for screening and verification. For EtG, add-ons, and/or tests exceeding 10 panels, a 150mg sample is ideal. We suggest weighing the sample using a jeweler’s scale. If scalp hair is unavailable, a similar quantity of body hair can be collected. Note, when referring to head hair, we mean scalp hair solely. Body hair denotes all other types (facial, axillary, etc.).

Process Overview

The key steps in the lab processing of a drug testing result include Accessioning, Screening, Extraction, and Confirmation.

Accessioning covers the sample's integration into a lab system. This step involves verifying that the sample was sealed and sent correctly, assigning a random LAN (Laboratory Accessioning Number), and completing any other data entry not handled by an electronic chain of custody system.

Screening provides an initial rapid evaluation for drug abuse. While Screening is an economical method to screen out drug usage in most samples, a positive screen needs confirmation to be valid in legal settings. Those samples that are presumptively positive in Screening require another level of confirmation.

If a sample shows a presumptive positive in the Screening stage, more hair is taken from the original sample and readied for Extraction. In this step, drugs are isolated from hair at lower concentrations compared to other methods (e.g., urine or oral fluid), which is why hair drug screening is notably challenging.

Confirmation of any positive screening outcome is carried out using GC/MS, GC/MS/MS or LC/MS/MS. All presumptive positive samples receive washing before confirmation as necessary. The complete lab process from Accessioning to Confirmation is examined under both the CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
