Hair analysis has emerged as a potent means for identifying drug and alcohol consumption. Hair captures a prolonged history of alcohol and drug usage by embedding biomarkers within the fibers of the growing hair thread. When taken near the scalp, hair can offer up to roughly a 3-month detection period for alcohol and drugs. Hair collection is easy, somewhat resistant to manipulation, and convenient for shipping.

A 1.5-inch portion of about 200 hair strands (comparable to the size of a #2 pencil) closest to the scalp yields 100mg of hair, which is the optimal sample for testing and verification. For EtG, additional inclusions, and/or assessments exceeding 10 panels, a specimen of 150mg is suggested. We advise measuring the specimen using a jeweler’s scale. If scalp hair cannot be sourced, a similar quantity of body hair may be obtained. When referencing head hair, it specifically denotes scalp hair. Body hair encompasses all other types (facial, axillary, etc.).

Process Summary

The four primary stages in the laboratory processing of a drug test result are Accessioning, Screening, Extraction, and Verification.

Accessioning entails the preliminary processing of a sample into a lab’s system. This process includes checking that the sample was correctly sealed and dispatched, assigning a random LAN (Laboratory Accessioning Number), and filling in any supplementary data not supplied by an electronic custody chain system.

Screening involves an initial rapid examination for abuse of substances. Although Screening offers an economical method to rule out drug usage in most samples, a positive screening must be confirmed to be permissible in court. Any samples that are presumptively positive in Screening necessitate a secondary confirmation.

If a sample tests presumptively positive during the Screening stage, additional hair is extracted from the initial specimen and readied for Extraction. During this phase, drugs are isolated from hair at a significantly lower concentration than in other techniques (such as urine or oral fluid), which is why hair drug testing is the most challenging technique to execute.

Confirmation of any positive screening outcome is performed through GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples are washed prior to confirmation if necessary. The entire laboratory process from Accessioning to Confirmation undergoes evaluation under both the CAP (College of American Pathologists) Hair criterion and the accreditation to ISO / IEC 17025 standards.
