If scalp hair is hard to obtain, an equivalent quantity of body hair is an option. When we mention head hair, we specifically mean scalp hair. Body hair includes any other hair type such as facial, axillary, etc.

Process Overview

The key four stages in the lab processing of a drug test outcome include Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the primary step of processing a sample into the lab system. This process ensures the sample's seal and shipment were intact, allocates a random LAN (Laboratory Accessioning Number), and fills in any additional data not captured by an electronic chain of custody.

Screening is a preliminary assessment for drug abuse detection. Although screening is an economical means to eliminate drug use in many cases, a positive finding requires further confirmation to be valid in legal contexts. Samples that show a potential positive in Screening must undergo secondary confirmation.

If a sample yields a potential positive result in the Screening phase, additional hair is taken from the initial batch for Extraction. At this juncture, drug concentrations are extracted at a lower concentration compared to other methods like urine or oral tests, making hair analysis a highly challenging process.

Positive screening outcomes are confirmed through methods like GC/MS, GC/MS/MS, or LC/MS/MS. All potential positive samples undergo necessary washing before the verification step. The complete sequence from Accessioning to Confirmation is reviewed under both CAP (College of American Pathologists) Hair guidelines and ISO / IEC 17025 accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
