A sample measuring 1.5 inches, consisting of roughly 200 hair strands (similar in size to a #2 pencil) nearest to the scalp will provide 100mg of hair, which is the optimal sample amount for testing and verification. For EtG, supplementary tests, and/or analyses exceeding 10 panels, a specimen amount of 150mg is advised. Utilizing a jeweler’s scale for weighing the specimen is recommended. If scalp hair isn’t available, an equivalent quantity of body hair may be used. When mentioning head hair, we refer specifically to scalp hair. Body hair denotes all other hair categories (facial, armpit, etc.).

Process Overview

The process of laboratory handling of a drug test outcome includes the four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves the primary processing of a sample within a laboratory's database. This encompasses confirming that the sample was appropriately sealed and dispatched, allocating a random LAN (Laboratory Accessioning Number), and finalizing any supplementary data entry not captured by an electronic chain of custody framework.

Screening involves a preliminary rapid assessment for drug abuse. Although Screening is efficient for eliminating drug use in most samples, a positive screen necessitates confirmation to be legally acceptable. Any samples tentatively positive in Screening do need an additional confirmation.

If a sample is tentatively positive during the Screening phase, extra hair is extracted from the initial specimen and readied for Extraction. In this step, drugs are removed from hair at significantly diminished concentrations compared to other methods (e.g., urine or saliva), which is why hair drug screening is the most challenging method to conduct.

Confirmation of any positive screening outcome is carried out using GC/MS, GC/MS/MS, or LC/MS/MS. All tentative positive samples undergo washing before confirmation if required. The entire lab procedure from Accessioning to Confirmation is examined under both the CAP (College of American Pathologists) Hair classification and the accreditation to ISO / IEC 17025 standards.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
