A 1.5-inch section containing roughly 200 hair strands (comparable to the width of a #2 pencil) near the scalp provides 100mg of hair, which is the optimal sample for both screening and verification. For tests pertaining to EtG, add-ons, or those exceeding 10 panels, a 150mg sample is advisable. Using a jeweler's scale is suggested for weighing the sample. If scalp hair is absent, a similar volume of body hair may be taken. By head hair, we mean exclusively scalp hair. Body hair encompasses all other kinds (such as facial or axillary).

Process Overview

The comprehensive lab procedure for producing a drug test result includes four phases: Accessioning, Screening, Extraction, and Confirmation.

Accessioning signifies the preliminary handling of a sample within a lab’s system. This phase involves checking that the sample is properly sealed and shipped, generating a unique LAN (Laboratory Accessioning Number), and completing any supplementary data entry not supplied by an electronic custody chain system.

Screening refers to an initial swift evaluation for illicit substances. While Screening is a budget-friendly method to exclude drug use on most samples, an affirmative screen must be verified for legal admissibility. Any samples that test presumptively positive during Screening require further validation.

If Screening indicates a presumptive positive, additional hair is taken from the original specimen for Extraction. This process extracts drugs at significantly lower concentrations than other approaches (such as urine or oral fluid), making hair drug analysis a challenging technique to execute.

Confirmation for any positive screen result is achieved with GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples undergo washing before confirmation as necessary. The entire process from Accessioning to Confirmation is subject to review under the CAP (College of American Pathologists) Hair label and the accreditation to ISO / IEC 17025 standards.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
