A hair sample measuring 1.5 inches with about 200 strands (akin to a #2 pencil) taken nearest to the scalp yields 100mg, an optimal amount for both screening and validation. For EtG, additional tests, and panels exceeding ten, 150mg of hair is suggested. It's advised to use a jeweler’s scale for measuring. If head hair is unavailable, an equal body hair sample may be taken. "Head hair" specifically refers to scalp hair, while "body hair" encompasses facial, axillary, and other types.

Process Overview

The processing of drug test results in a laboratory involves four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial step where a sample is introduced into a laboratory's system. This involves ensuring the sample is properly sealed and transported, assigning a unique LAN (Laboratory Accessioning Number), and completing any remaining data entry absent in an electronic chain of custody system.

Screening serves as a preliminary evaluation for drug presence. Though Screening is an economical method to rule out drug usage in most cases, any positive screening requires subsequent confirmation to be valid in a legal setting. Any samples testing tentatively positive during Screening necessitate a second confirmation step.

In instances of tentative positive results during Screening, additional hair is extracted from the initial sample for further preparation in the Extraction phase. Here, drugs are extracted from hair at lower concentrations compared to other methods, which is why hair drug screening is considered technically challenging.

Confirmation of any positive results from Screening is performed using GC/MS, GC/MS/MS, or LC/MS/MS techniques. All samples testing tentatively positive are cleansed before confirmation if required. The lab process from Accessioning to Confirmation is scrutinized under both the CAP (College of American Pathologists) Hair guidelines and the ISO / IEC 17025 accreditation standards.

The impact of hair treatments on the oxidation/decomposition of drugs in hair was also explored, as the long-term fate of drugs incorporated in the keratin matrix. The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
