What it is, How it works

Hair is straightforward to gather, relatively tough to tamper with, and simple to transport.

A 1.5-inch segment of roughly 200 hair strands (comparable in size to a #2 pencil) near the scalp will yield 100mg of hair, which is the optimal amount for analysis and confirmation. For expanded panels such as EtG add-ons or those exceeding 10 panels, a 150mg sample is suggested. We advise that the sample be weighed using a jeweler’s scale. In the absence of scalp hair, a similar quantity of body hair can be utilized. In the context of head hair, we specifically mean scalp hair. Body hair covers all other types (facial, axillary, etc.).

Process Summary

The principal stages in processing a laboratory drug test result include Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial handling of a sample into the laboratory’s system. It involves ensuring the sample was correctly sealed and delivered, assigning a unique LAN (Laboratory Accessioning Number), and finishing any supplementary data entry that is not included in an electronic chain of custody system.

Screening is a preliminary rapid assessment for identifying drugs of abuse. Although Screening is a cost-efficient method to eliminate drug usage in most samples, a positive screen needs verification for legal admittance. Any samples that show tentative positive results in Screening require a subsequent confirmation.

If a sample is tentatively positive during Screening, additional hair is taken from the original sample for Extraction preparation. At this stage, drugs are extracted from hair at a much lower concentration compared to other methods (such as urine or oral fluid), making hair drug testing the most challenging assay to conduct.

Confirmation for any initial positive Screening results is performed using GC/MS, GC/MS/MS or LC/MS/MS techniques. All presumed positive samples are cleansed before confirmation if necessary. The complete lab process from Accessioning to Confirmation is audited under both the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 standards accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
