What it is, How it works

Hair testing is regarded as an effective method for detecting drug and alcohol consumption. Hair records a prolonged history of alcohol and drug usage by capturing biomarkers within the strands of the growing hair. When sampled near the scalp, hair can offer a detection window of up to roughly 3 months for alcohol and other substances. Hair is easy to gather, relatively hard to tamper with, and straightforward to transport.

A section of hair measuring 1.5 inches, comprising around 200 strands (approximately the size of a #2 pencil) nearest to the scalp, yields 100mg of hair, the optimal sample for both screening and validation. For EtG, add-ons, and/or tests exceeding 10 panels, a sample of 150mg is suggested. We advise measuring the specimen with a jeweler’s scale. If scalp hair is not available, a comparable quantity of body hair may be collected. When we mention head hair, we mean scalp hair specifically. Body hair includes all other types (facial, axillary, etc.).

Process Overview

The processing of a drug test result in the lab involves four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the preliminary processing of a sample into a lab’s system. This step verifies the sample was properly sealed and shipped, assigns a random LAN (Laboratory Accessioning Number), and completes any data entry not already provided by an electronic chain of custody system.

Screening is a preliminary quick examination for drugs of abuse. While Screening is a cost-efficient method to eliminate drug usage on most samples, a positive screening must be verified to be accepted in court. Samples that are preliminarily positive during Screening need a secondary confirmation.

If a sample tests preliminarily positive at the Screening phase, more hair is extracted from the initial sample and prepared for Extraction. In this stage, drugs are removed from hair at much reduced concentrations compared to other methods (e.g., urine or oral fluid), hence hair drug screening is recognized as the most complex methodology.

Verification of a positive screening outcome is performed using GC/MS, GC/MS/MS, or LC/MS/MS. All samples with presumptive positives are cleansed before confirmation as needed. The complete lab process from Accessioning to Confirmation is examined under both the CAP (College of American Pathologists) Hair designation, along with the ISO / IEC 17025 accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
