Collecting hair is straightforward, it is fairly resistant to tampering, and it is uncomplicated to transport.

A segment measuring 1.5 inches with around 200 strands (approximately the thickness of a #2 pencil) situated nearest to the scalp should provide a 100mg sample, which is optimal for analysis. For EtG, additional tests, and/or panels exceeding ten, a specimen of 150mg is advised. It is suggested to weigh the specimen using a jeweler’s scale. If scalp hair is not accessible, a comparable quantity of body hair can be used. "Head hair" refers exclusively to scalp hair, while "body hair" encompasses all other types (such as facial, axillary, etc.).

Process Overview

The key phases in the lab processing of a drug testing sample include Accessioning, Screening, Extraction, and Confirmation.

Accessioning consists of the preliminary handling of a sample into a lab’s framework. This process verifies the proper sealing and shipping of the sample, allocates a random LAN (Laboratory Accessioning Number), and fulfills any remaining data entry absent from an electronic chain of custody system.

Screening provides a preliminary evaluation for substance misuse. Although Screening economically dismisses drug use in most cases, a confirmed positive is necessary for judicial acceptance. Samples yielding a presumptive positive during Screening require further validation.

With a presumptive positive during Screening, additional hair from the initial sample is used for Extraction. Here, substances are extracted from hair at significantly lesser concentrations compared to other techniques (e.g., urine or oral fluid), thus making hair drug screening the most challenging to execute.

Any positive results from Screening undergo Confirmation using GC/MS, GC/MS/MS, or LC/MS/MS. Presumptive positive samples are washed prior to confirmation as deemed necessary. The entire lab procedure from Accessioning to Confirmation is scrutinized under both the CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 standard accreditation.
