If scalp hair isn't available, an equivalent quantity of body hair can be obtained. When we mention head hair, it refers specifically to scalp hair. Body hair covers all other types (facial, axillary, etc.).

Process Overview

The primary steps in processing a drug test in the lab are Accessioning, Screening, Extraction, and Confirmation.

Accessioning means the initial incorporation of a sample into the lab’s system. This entails checking that the sample was correctly sealed and dispatched, generating a random LAN (Laboratory Accessioning Number), as well as fulfilling any extra data input not automatically handled by an electronic chain of custody system.

Screening provides an initial rapid assessment for substances of abuse. Though Screening is an economical method to dismiss drug usage for most samples, a positive result must be confirmed for legal admissibility. All samples flagged as potentially positive in Screening necessitate additional verification.

When a sample's Screening stage implies positivity, further hair from the initial batch undergoes preparation for Extraction. This phase retrieves drugs from hair at a much finer concentration than other processes (e.g., urine or oral fluid), making hair testing the most intricate method to execute.

Verification of any positive screening outcomes is carried out using GC/MS, GC/MS/MS, or LC/MS/MS. Before confirming, all likely positive samples go through a cleansing rinse as required. The entire lab procedure from Accessioning through to Confirmation is evaluated under the CAP (College of American Pathologists) Hair criteria and the ISO / IEC 17025 standards.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
