For EtG, additional tests, and/or tests greater than 10 panels, 150mg of the sample is advised. We suggest using a jeweler’s scale to weigh the sample. If scalp hair isn't available, an equivalent amount of body hair can be obtained. The term head hair strictly means scalp hair. Body hair refers to all other sorts of hair (facial, axillary, etc.).

Process Overview

The four primary steps in laboratory processing for drug testing results include Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the inaugural processing that inserts a sample into a lab’s system. It involves confirming that the sample was sealed and mailed correctly, assigning a unique LAN (Laboratory Accessioning Number), and completing any outstanding data entry not supplied by an electronic chain of custody.

Screening consists of a preliminary brief assessment for abuse drugs. Screening is cost-effective for eliminating drug use on most samples; however, a positive screen must be confirmed to be court-admissible. Samples yielding presumptive positives in Screening do require further confirmation.

If a sample appears positive initially in Screening, more hair is extracted from the initial sample and set for Extraction. In this procedure, drugs are retrieved from hair at a significantly lower concentration than in other methods (e.g., urine or oral fluid), explaining why hair drug testing is the most intricate approach to execute.

Affirmation of a positive screening outcome undergoes GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples undergo cleansing before confirmation as needed. The comprehensive laboratory procedure from Accessioning to Confirmation is scrutinized under both the CAP (College of American Pathologists) Hair designation and accreditation to the ISO/IEC 17025 benchmarks.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
