For EtG, supplementary tests, and/or panels beyond 10, 150mg of the sample is advisable. It's suggested to measure the sample with a jeweler's scale. If scalp hair cannot be sourced, a comparable quantity of body hair may be taken. Head hair pertains solely to scalp hair. Body hair includes all other types (facial, axillary, etc.).

Process Overview

The four primary stages involved in the laboratory processing of a drug test are Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves the sample's initial processing into a laboratory's framework. This entails confirming that the sample was properly sealed and dispatched, assigning a random LAN (Laboratory Accessioning Number), and fulfilling any additional data entry absent from an electronic chain of custody system.

Screening entails a preliminary rapid check for substance misuse. Samples yielding presumptively positive results in Screening necessitate additional confirmation.

When a sample registers presumptively positive during the Screening phase, additional hair is extracted from the initial specimen and prepared for Extraction. At this juncture, substances are retrieved from hair at significantly lower concentrations than other methodologies (e.g., urine or oral fluid), contributing to the complexity of hair drug screening.

Confirmation of any positive Screening outcome is conducted using GC/MS, GC/MS/MS or LC/MS/MS. All samples presumed positive are cleansed prior to confirmation as required. The complete laboratory process from Accessioning to Confirmation undergoes review under both the CAP (College of American Pathologists) Hair designation and accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
