If scalp hair isn’t accessible, a similar amount of body hair can be used. When referring to head hair, we mean scalp hair only. Body hair includes all other hair types (facial, axillary, etc.).

Process Overview

The four core steps involved in the lab processing of a drug test result are Accessioning, Screening, Extraction, and Confirmation.

Accessioning includes the initial processing of a specimen into the lab's system. This step verifies that the specimen was correctly sealed and sent, assigns a random LAN (Laboratory Accessioning Number), and completes any supplementary data input not available from an electronic chain of custody system.

Screening comprises an initial quick check for drugs of abuse. Although Screening offers a cost-effective strategy to rule out drug usage in most specimens, a positive screen must be confirmed for court admittance. Specimens that are presumptively positive in Screening do require a secondary verification.

If a specimen is presumptively positive in the Screening phase, additional hair is extracted from the initial sample and prepped for Extraction. Here, drugs are extracted from hair at lower concentrations compared to other methods (like urine or oral fluid), making hair drug screening the most challenging methodology to execute.

Verification of any positive screening outcome is executed via GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples are washed before confirmation as needed. The entire lab process from Accessioning to Confirmation is reviewed under both the CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
