What it is, How it works

Hair offers an extended record of alcohol and drug use by encasing biomarkers within the fibers of the hair as it grows. When strands are collected close to the scalp, they can reveal up to a 90-day history of alcohol and drug use. Hair collection is straightforward, relatively resistant to tampering, and convenient for transportation.

A 1.5-inch segment comprising approximately 200 hair strands (equivalent to the thickness of a #2 pencil) nearest to the scalp produces 100mg of hair, serving as the optimal sample for both screening and confirmation. For EtG, additional analyses, and/or testing panels exceeding ten, a sample weight of 150mg is advised. Utilizing a jeweler’s scale for specimen measurement is recommended. Should scalp hair be inaccessible, a proportionate amount of body hair is accepted. When referring to head hair, it denotes scalp hair exclusively. Body hair encompasses all other variations (facial, axillary, etc.).

Process Overview

The processing of a drug test result in a laboratory involves four steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning begins with the initial integration of the sample into the lab’s system. This phase checks that the sample was correctly sealed and sent, assigns a random LAN (Laboratory Accessioning Number), and completes any leftover data entry missing from an electronic chain of custody system.

In the Screening phase, a swift evaluation for drugs of abuse occurs. Although Screening is a cost-beneficial method for eliminating drug use in most samples, a positive screen needs verification to be accepted in a court setting. Samples that test presumptively positive in Screening require a subsequent confirmation.

If presumed positive at the Screening phase, an additional portion of hair from the original sample undergoes preparation for Extraction. Here, drugs are extracted from hair at notably lower concentrations compared to other methods (i.e., urine or oral fluids), making hair drug screening the most challenging methodology to conduct.

Confirmation of screening findings uses either GC/MS, GC/MS/MS, or LC/MS/MS. All samples deemed presumptively positive undergo washing before confirmation, as necessary. The entire laboratory process from Accessioning through Confirmation receives assessment under both the designation by CAP (College of American Pathologists) for Hair and accreditation to the ISO / IEC 17025 standards.

Advantages of hair drug testing:

Clinical Applications of hair testing are witnessed by the insight on environmental tobacco smoke exposure, assessed by hair analysis in a population of students and the study of in utero-exposure to drugs of abuse through neonatal hair analysis. Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
