Hair analyses have gained recognition as a potent method for identifying substance and alcohol intake. Hair serves as a chronicled indicator of alcohol and drug consumption by encapsulating biomarkers in the strands as they grow. When collected near the scalp, hair can offer an approximate 3-month detection period for substances and alcohol. Hair samples are easy to collect, relatively challenging to tamper with, and convenient to transport.
A sample measuring 1.5 inches of roughly 200 hair strands (akin to a #2 pencil in thickness) near the scalp will provide 100mg of hair, which is the preferred quantity for both screening and confirmation procedures. For enhanced screens such as EtG, additional panels exceeding 10, it is advisable to collect 150mg of the sample. It's advisable to use a jeweler’s scale for precise weighing of the sample. If scalp hair is not available, an equivalent quantity of body hair can be utilized. In reference to head hair, this pertains solely to scalp hair. Body hair denotes all other types like facial or axillary hair.
Process Overview
There are four primary stages in the lab processing of a drug test outcome: Accessioning, Screening, Extraction, and Confirmation.
Accessioning is the preliminary phase where a sample is integrated into a laboratory’s system. This involves confirming the sample’s sealing and shipping integrity, assigning it a random Laboratory Accessioning Number (LAN), and completing any additional data entries not already included in an electronic custody system.
Screening comprises an initial review for substances of abuse. While Screening is an economical method to dismiss drug usage in most samples, positive screens require confirmation to be legally defensible. Samples that are likely positive after Screening need a confirmatory test.
Should a sample seem positive during Screening, further hair is extracted from the original specimen and prepared for Extraction. During this stage, substances are extracted from hair at much reduced concentrations compared to other methods like urine or oral fluid, making hair drug testing most challenging to execute.
Confirming a positive screening result is carried out via GC/MS, GC/MS/MS, or LC/MS/MS techniques. All suspected positive samples undergo a necessary wash prior to Confirmation. The whole laboratory sequence from Accessioning through Confirmation is examined under the CAP (College of American Pathologists) Hair designation, alongside compliance with ISO / IEC 17025 standards.
