What it is, How it works

Hair testing is recognized as an effective method for identifying drug and alcohol consumption. Hair captures biomarkers within its fibers, offering a long-term record of substance use. Collected near the scalp, hair can exhibit a detection window for alcohol and drugs of approximately three months. Hair is easy to collect, somewhat resistant to tampering, and convenient to transport.

A 1.5-inch sample of about 200 hair strands (similar to the thickness of a #2 pencil) near the scalp yields 100mg of hair, the optimal amount for screening and confirmation. For EtG tests, add-ons, and/or tests beyond 10 panels, 150mg of the specimen is advised. Using a jeweler's scale to weigh the specimen is recommended. If scalp hair is not available, an equivalent amount of body hair can be collected. When we mention head hair, we mean scalp hair exclusively, while body hair includes all other hair types (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result involves four main phases: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the preliminary step of entering a sample into a lab’s system, involving confirmation of a properly sealed and shipped sample, assigning a random LAN (Laboratory Accessioning Number), and completing additional data entry that might be absent from an electronic chain of custody system.

Screening is an initial, rapid evaluation for drugs of abuse. It is a cost-efficient method to exclude drug use for most samples, but a positive screen must be verified for court admissibility. Samples that test presumptively positive in Screening need a secondary confirmation.

If a sample is presumptively positive during Screening, additional hair is extracted from the original specimen for the Extraction phase. In this process, drugs are extracted from hair at a much lower concentration compared to other methods (e.g., urine or oral fluid), making hair drug testing the most challenging methodology.

Confirmation of any positive screen is conducted through GC/MS, GC/MS/MS, or LC/MS/MS. Presumptive positive samples undergo washing before confirmation if necessary. The complete laboratory procedure from Accessioning to Confirmation is assessed under both the CAP (College of American Pathologists) Hair classification and the ISO / IEC 17025 standards accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field.
