A segment measuring 1.5 inches from around 200 hair strands (approximately the width of a #2 pencil) near the scalp yields 100mg, which is optimal for screening and confirmation. For EtG, additional tests, and protocols beyond 10 panels, a specimen of 150mg is suggested. It’s advisable to measure the sample using a jeweler’s scale. If scalp hair isn’t available, an equivalent quantity of body hair might be used. By head hair, we mean scalp hair only, whereas body hair encompasses other types of hair (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test involves four critical stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning refers to the preliminary handling of a sample in a lab's system, which includes confirming proper sealing and shipping of the sample, assigning a unique LAN (Laboratory Accessioning Number), and inputting any additional information not covered by an electronic chain of custody.

Screening constitutes an initial assessment to check for drugs of abuse. Though it efficiently rules out drug usage in many samples, any positive screen requires confirmation for legal acceptability. Any samples detected as presumptively positive must undergo secondary confirmation.

If a sample tests presumptively positive in Screening, additional hair is obtained from the original specimen for Extraction. At this phase, substances are isolated from hair at a much lower concentration compared to other methods like urine or oral fluid, highlighting why hair drug testing is particularly complex.

Confirmation of a positive screen is performed using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive specimens are cleansed before confirmation as necessary. The complete procedure, from Accessioning to Confirmation, is examined according to both CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 standards.
