If scalp hair isn’t available, a comparable quantity of body hair can be collected. Reference to head hair pertains exclusively to scalp hair. Body hair encompasses all other types of hair (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result involves four central steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the initial recording of a sample into the laboratory’s system. This step ensures that the sample was correctly sealed and shipped, assigns a random LAN (Laboratory Accessioning Number), and completes any extra data entry that an electronic chain of custody system might not provide.

Screening includes a preliminary rapid assessment for drugs of misuse. While Screening is a cost-efficient approach to exclude drug usage in most samples, a positive result needs to be verified for legal acceptance. Presumptively positive samples in Screening require further confirmation.

Should a sample be presumptively positive at the Screening stage, additional hair is extracted from the initial sample for the Extraction process. Here, drugs are isolated from hair at a much lower concentration compared to other techniques (such as urine or oral fluid), which makes hair drug screening the most challenging method to execute.

Confirmation of any positive Screening result is conducted through GC/MS, GC/MS/MS, or LC/MS/MS. Any presumptive positive samples are cleansed before confirmation as needed. The complete laboratory process from Accessioning to Confirmation is evaluated under both the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
