What it is, How it works

When hair is collected near the scalp, it offers a detection period for alcohol and drugs of up to roughly 90 days. Hair collection is straightforward, relatively resistant to tampering, and easy to transport.

A sample measuring 1.5 inches consisting of about 200 hair strands (the size of a #2 pencil) taken closest to the scalp yields 100mg of hair, which is optimal for both screening and confirmation purposes. For testing including EtG, add-ons, or beyond 10 panels, a specimen of 150mg is advisable. The specimen should be weighed using a jeweler’s scale. Should scalp hair be inaccessible, an equivalent weight of body hair can be used. "Head hair" refers exclusively to scalp hair, whereas "body hair" encompasses all other hair types, including facial and axillary hair.

Process Overview

The four core steps in laboratory drug test processing are Accessioning, Screening, Extraction, and Confirmation.

During Accessioning, samples undergo initial processing into the lab's system. This stage ensures the sample was sealed and sent correctly, assigns a random LAN (Laboratory Accessioning Number), and fills in any missing information that wasn't provided digitally by a chain of custody system.

The Screening step involves an initial assessment for drugs of abuse. Screening is a cost-efficient means to dismiss drug usage for the majority of samples; however, any positive results from Screening must be verified to be court-admissible. Samples that are presumptively positive during Screening must undergo further Confirmation.

Confirmation for any positive Screening outcome is conducted using GC/MS, GC/MS/MS, or LC/MS/MS techniques. All presumptive positive samples undergo washing before Confirmation when necessary. The complete laboratory process from Accessioning to Confirmation is evaluated according to both CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
