What it is, How it works

When hair is obtained near the scalp, it offers a detection window for substances up to around 3 months. Hair collection is straightforward, relatively tamper-resistant, and convenient for shipping.

A sample size of 1.5 inches comprising approximately 200 hair strands (similar in size to a #2 pencil) near the scalp yields 100mg, which is optimal for screening and confirmation processes. For EtG, additional analyses, and/or screenings beyond 10 panels, a specimen of 150mg is suggested. It's advisable to measure the specimen using a jeweler's scale. If scalp hair is not an option, the equivalent amount of body hair may be used. When mentioning head hair, we specifically mean scalp hair. Body hair encompasses all other hair types (facial, axillary, etc.).

Process Overview

The laboratory drug test analysis encompasses four key phases: Accessioning, Screening, Extraction, and Confirmation.

During Accessioning, a sample is initially processed into the laboratory's system. This includes verifying proper sealing and shipment of the sample, assigning a random LAN (Laboratory Accessioning Number), and completing any extra data input not managed by an electronic chain of custody system.

Screening acts as an initial rapid check for substances of abuse. Though Screening is an economical method to exclude substance use in most samples, a positive result requires confirmation to be valid in legal settings. Samples that show presumptive positives in Screening need to undergo further confirmation.

If a sample presents a presumptively positive result in the Screening phase, additional hair is extracted from the initial specimen and prepared for Extraction. At this stage, drugs are drawn from hair at a much lower concentration compared to other methods (e.g., urine or oral fluid), making hair drug screening the most challenging method to execute.

Any positive result from Screening is verified using GC/MS, GC/MS/MS or LC/MS/MS. All samples presumed positive undergo washing prior to confirmation if necessary. The complete laboratory process, from Accessioning to Confirmation, adheres to the CAP (College of American Pathologists) Hair designation and is accredited to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
