What it is, How it works

When collected near the scalp, hair can offer approximately a 3-month window for detecting alcohol and other drugs. It is straightforward to collect, somewhat resistant to adulteration, and simple to ship.

A 1.5-inch sample of roughly 200 hair strands (comparable to the size of a #2 pencil) nearest to the scalp will yield 100mg of hair, the ideal quantity for screening and confirmation. For EtG, add-ons, and/or tests exceeding 10 panels, 150mg of the sample is advised. We suggest weighing the sample using a jeweler’s scale. If scalp hair is not available, a similar amount of body hair may be obtained. When mentioning head hair, we mean scalp hair exclusively. Body hair pertains to all other hair types (such as facial, axillary, etc.).

Process Overview

The four principal steps in the lab processing of a drug test result are Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves the initial integration of a sample into a laboratory’s system. This covers confirming that the sample was sealed and transported correctly, assigning a random LAN (Laboratory Accessioning Number), and completing any extra data entry absent from an electronic chain of custody system.

Screening provides a preliminary quick assessment for drugs of abuse. Although Screening is a cost-effective means to exclude drug use on most samples, a positive screen requires confirmation to be admissible in court. Any samples that are presumptively positive during Screening necessitate secondary confirmation.

If a sample tests presumptively positive in the Screening phase, additional hair is extracted from the initial specimen and readied for Extraction. Here, drugs are extracted from the hair at a considerably lower concentration than in other methods (e.g., urine or oral fluid), which accounts for hair drug screening being the most challenging method to execute.

Confirmation of any positive screening result is undertaken using GC/MS, GC/MS/MS or LC/MS/MS. All presumptive positive samples are cleansed prior to confirmation if needed. The entire lab process from Accessioning to Confirmation is scrutinized under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
