What it is, How it works

When obtained near the scalp, hair can offer a detection period for alcohol and other drugs extending up to roughly three months. Collecting hair is straightforward, challenging to tamper with, and convenient to transport.

A 1.5-inch section comprising around 200 strands of hair (comparable to a #2 pencil's circumference) nearest to the scalp yields 100mg of hair, which is optimal for both screening and confirmation. For EtG, add-ons, and/or tests exceeding 10 panels, a sample of 150mg is advisable. It is suggested to weigh the hair on a jeweler’s scale. Should scalp hair be unavailable, an equivalent volume of body hair may be used. When mentioning head hair, it pertains solely to scalp hair. Body hair encompasses all other types (facial, axillary, etc.).

Process Overview

The core stages in the laboratory analysis of a drug test result include Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves initially processing a sample into the lab's system. This step verifies the sample’s proper sealing and shipping, assigns a random LAN (Laboratory Accessioning Number), and completes any extra data entry not covered by an electronic chain of custody system.

Screening performs an initial rapid drug abuse check. Although Screening is a cost-efficient way to eliminate drug use in most samples, a positive screen must be confirmed for court admissibility. Samples presumed positive in Screening necessitate a secondary confirmation.

If a sample is presumed positive during Screening, additional hair is taken from the original specimen for Extraction. Here, drugs are separated from the hair at a considerably lower concentration than found in other methods (e.g., urine or oral fluid), which is why hair drug screening is considered the most challenging method.

Confirmation of any positive screening result is executed using GC/MS, GC/MS/MS, or LC/MS/MS. All samples presumed positive undergo washing before confirmation as required. The complete lab process, from Accessioning to Confirmation, is reviewed under the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
