What it is, How it works

When collected from the scalp, hair can indicate alcohol and drug use for a span of around 3 months. It is simple to gather, hard to tamper with, and straightforward to transport.

Taking a 1.5-inch sample composed of approximately 200 hair strands (about the width of a #2 pencil) nearest to the scalp provides 100mg of hair, which is the optimal quantity for screening and confirmation. For EtG, additional panels, and/or tests exceeding 10 panels, 150mg of the specimen is suggested. We advocate for using a jeweler’s scale to measure the specimen. Should scalp hair be unavailable, an equal quantity of body hair may be used. Our mention of head hair exclusively pertains to scalp hair. All other hair types (facial, axillary, etc.) are classified as body hair.

Process Overview

The laboratory processing of a drug test result consists of four principal steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves the preliminary processing of a sample into a lab’s system. This process ensures that the sample was properly sealed and shipped, allocates a random LAN (Laboratory Accessioning Number), and completes any remaining data entry that is not covered by an electronic chain of custody setup.

Screening is an initial rapid test for identifying drug abuse. Although Screening is an economical way to dismiss drug consumption for most samples, any positive results must be confirmed to hold in legal proceedings. Samples with presumptive positive results in Screening require additional confirmation.

Upon yielding a presumptive positive in the Screening phase, additional hair is extracted from the initial sample and readied for Extraction. This phase involves extracting drugs from hair at a concentration significantly lower than other techniques (e.g., urine or oral fluids), highlighting why hair drug screening is the most challenging methodology to execute.

Confirmation of a positive Screening outcome is conducted via GC/MS, GC/MS/MS, or LC/MS/MS. All samples with presumptive positive results are cleansed before confirmation if necessary. The entire laboratory procedure from Accessioning to Confirmation is assessed under both the CAP (College of American Pathologists) Hair designation and accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
