What it is, How it works

Hair testing is increasingly recognized as a formidable means for uncovering drug and alcohol consumption. Hair captures a prolonged history of alcohol and other drugs by encasing biomarkers within the fibers of the growing strand. When taken from near the scalp, hair can provide up to a 3-month detection timeframe for alcohol and other substances. Hair is effortless to gather, relatively resistant to adulteration, and convenient to transport.

A 1.5-inch segment comprising about 200 hair strands (approximately the size of a #2 pencil) nearest to the scalp will yield 100mg of hair, which is the preferred sample for both screening and confirmation. For EtG, add-ons, and/or tests exceeding 10 panels, 150mg of the sample is advised. It's suggested to weigh the sample using a jeweler’s scale. If scalp hair isn't available, an equivalent measure of body hair can be obtained. When discussing head hair, we are specifically addressing scalp hair. Body hair indicates all other hair varieties (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result involves four key steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the source processing of a sample within a lab’s framework. This step includes confirming that the sample was appropriately sealed and mailed, allocating a random LAN (Laboratory Accessioning Number), and completing any additional data recording not provided by an electronic chain of custody protocol.

Screening entails an initial rapid assessment for drugs of abuse. Although Screening is an economical approach to exclude drug usage for most samples, a positive result in the Screening demands verification to be permissible in legal proceedings. Samples displaying presumptive positives in Screening necessitate an additional confirmation.

If a sample shows a presumptive positive during the Screening phase, additional hair is retrieved from the primary sample and readied for Extraction. Here, drugs are extracted from hair at notably lesser concentrations than in other methodologies (such as urine or oral fluid), underscoring why hair drug testing is the most challenging methodology to execute.

Verification of any positive screening outcome is conducted using GC/MS, GC/MS/MS, or LC/MS/MS. Any samples showing presumptive positive results are cleansed prior to confirmation if essential. The comprehensive lab procedure from Accessioning to Confirmation is evaluated under both the CAP (College of American Pathologists) Hair endorsement and the certification to ISO / IEC 17025 standards.

Benefits of hair drug testing:

  • Extended detection timeframe: Hair drug tests can identify drug use for up to 90 days, unlike urine tests which offer a shorter detection span.
  • Challenges in evasion: It is exceptionally difficult to manipulate a hair drug test, ensuring more reliable outcomes.
  • Delivers historical insights: This method can reveal drug use trends over time, not merely recent consumption.

Drawbacks:

  • Inability to identify recent use: It takes around 5-7 days for drugs to become traceable in hair.
  • Expense: Hair drug tests are typically pricier than other drug screening methods.
  • Result variations: Factors like hair color and individual differences in hair growth can influence the concentration of drug metabolites present in the hair.

Note: Although often termed as "hair follicle tests", the test actually examines the hair strand rather than the follicle below the scalp.
