What it is, How it works

Hair testing is now recognized as a potent method for identifying drug and alcohol usage. When gathered near the scalp, hair can present a detection timeframe of up to roughly 3 months for alcohol and other drugs. Hair collection is uncomplicated, hard to tamper with, and straightforward to transport.

A 1.5-inch segment consisting of approximately 200 hairs (resembling the size of a #2 pencil) closest to the scalp will yield 100mg of hair, which is the perfect amount for screening and verification. For tests like EtG, add-ons, and/or those exceeding 10 panels, 150mg of the sample is advisable. Measuring the specimen using a jeweler’s scale is recommended. If scalp hair is missing, an equivalent quantity of body hair can be collected. When discussing head hair, it pertains specifically to scalp hair. Body hair includes all other hair types (such as facial, axillary, etc.).

Process Overview

The four essential stages in processing a drug test result in the laboratory are Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the step where a sample is initially processed within a lab’s system. It involves confirming that the sample was correctly sealed and dispatched, allocating a unique LAN (Laboratory Accessioning Number), and filling in any missing data not provided by an electronic chain of custody system.

Screening consists of an initial swift examination for drugs of abuse. Though Screening is a cost-efficient approach to exclude drug use in most samples, a positive screen needs confirmation for it to be valid in court. Any samples yielding a presumptive positive in Screening necessitate further confirmation.

If a sample is presumptively positive during Screening, more hair is extracted from the original sample and prepped for Extraction. In this process, drugs are extracted from hair at a markedly lower concentration than with other methods (e.g., urine or oral fluid), which is why hair drug tests are the most challenging methodology to execute.

Confirmation of any positive screening outcome is carried out using GC/MS, GC/MS/MS, or LC/MS/MS. All samples that are presumptively positive undergo washing before confirmation as required. The entire lab procedure from Accessioning to Confirmation is evaluated under the CAP (College of American Pathologists) Hair designation and aligned with ISO / IEC 17025 standards.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
