Process Overview

The laboratory process for a drug test result comprises four key steps: Accessioning, Screening, Extraction, and Confirmation.

In Accessioning, the sample first enters the laboratory system. This procedure involves confirming proper sealing and shipping of the sample, assigning a unique LAN (Laboratory Accessioning Number), and completing any data entry that isn’t provided by an electronic chain of custody system.

Screening serves as an initial rapid test for drug abuse. While effective for eliminating drug use in most cases, a positive screen must be confirmed for court acceptability. Samples showing presumptive positivity in Screening require a secondary confirmatory procedure.

Upon presumptive positivity in the Screening phase, additional hair is extracted from the initial specimen and readied for Extraction. This stage involves isolating drugs from hair at significantly lower concentrations than those in other methodologies (e.g., urine or oral fluid), hence hair drug screening remains the most challenging technique to execute.

Positive Screening results undergo Confirmation via GC/MS, GC/MS/MS, or LC/MS/MS. All presumptively positive samples are cleansed before confirmation as necessary. The complete lab process from Accessioning to Confirmation is performed under both the CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 standard accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
