A 1.5-inch segment of roughly 200 hair strands (around the diameter of a #2 pencil) nearest the scalp yields 100mg of hair, which is the optimal amount for screening and confirmation. For EtG, additional analyses, and/or panels exceeding 10, it is recommended to gather 150mg of the sample. Using a jeweler’s scale is suggested for weighing the sample. If scalp hair is not present, an equivalent quantity of body hair may be used. In terms of head hair, this pertains exclusively to scalp hair. Body hair encompasses all other hair types (such as facial, axillary, etc.).

Process Overview

The laboratory procedure for processing a drug test result is composed of four primary phases: Accessioning, Screening, Extraction, and Confirmation.

Accessioning includes the preliminary processing of a sample into a laboratory’s system. This step ensures that the sample was correctly sealed and shipped, assigns an arbitrary LAN (Laboratory Accessioning Number), and completes any other data entry absent from an electronic chain of custody system.

Screening offers a rapid preliminary assessment for drug abuse. Although Screening is an economical option to eliminate drug use for the majority of samples, a positive screen result must be verified for court acceptability. Any samples that test presumptively positive in Screening need a subsequent confirmation.

If a sample shows a presumptive positive in the Screening phase, further hair is taken from the original sample to prepare it for Extraction. During this phase, drugs are extracted from hair at a lower concentration compared to other methods (such as urine or oral fluid), hence making hair drug screening the most complex method to carry out.

Positive results from a screening are verified through GC/MS, GC/MS/MS, or LC/MS/MS. All samples with presumptive positives undergo washing prior to confirmation when necessary. The complete process from Accessioning to Confirmation undergoes review under both the CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 standard accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
