A 1.5-inch segment of roughly 200 hair strands (comparable to the thickness of a #2 pencil) closest to the scalp will yield 100mg of hair, which is the preferred sample for analysis and verification. For EtG, additional tests, or tests exceeding 10 panels, a 150mg specimen is recommended. We suggest using a jeweler’s scale to weigh the sample. If scalp hair is unavailable, an equivalent quantity of body hair can be collected. Reference to head hair pertains to scalp hair only, while body hair includes any other hair type (facial, axillary, etc.).

Process Overview

The drug testing in laboratories involves four principal steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the introductory process of incorporating a sample into a lab’s system. It involves assuring the sample is sealed and shipped accurately, generating a random LAN (Laboratory Accessioning Number), and completing any supplementary data entry not included by an electronic chain of custody system.

Screening is an initial rapid assessment for drugs of abuse. Although Screening is an economical approach to exclude drug usage for most samples, positive results require confirmation for legal admissibility. Any samples that yield a presumptive positive outcome in Screening must undergo secondary confirmation.

In instances of presumptive positive results during Screening, additional hair is extracted from the primary specimen for the Extraction phase. In this step, drugs are retrieved from hair at substantially lower concentrations than other practices (e.g., urine or oral fluid), which is why hair drug testing is among the most challenging methodologies.

Confirmation of any positive Screening result is achieved via GC/MS, GC/MS/MS, or LC/MS/MS. Before confirmation, all presumptive positive samples are cleansed as required. The full laboratory procedure from Accessioning to Confirmation undergoes assessment under both the CAP (College of American Pathologists) Hair designation and the authorization to ISO / IEC 17025 standards.
