A hair sample measuring 1.5 inches, consisting of around 200 hair strands (similar to the thickness of a #2 pencil), obtained closest to the scalp, will yield 100mg of hair—ideal for both screening and confirmation. For EtG, additional tests, and/or tests exceeding 10 panels, a 150mg specimen is advised. We suggest using a jeweler’s scale to weigh the specimen. If scalp hair is insufficient, an equivalent quantity of body hair may be gathered. When referencing head hair, it pertains only to scalp hair. Body hair encompasses all other hair varieties (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result comprises four primary stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial processing of a sample within the laboratory system. This step verifies proper sealing and shipment of the sample, allocates a random LAN (Laboratory Accessioning Number), and completes any additional data entry not covered by an electronic chain of custody system.

Screening offers an initial rapid assessment for drugs of abuse. While Screening provides a cost-effective means to rule out drug use in most samples, court-admissible results require confirmation of any positive screen. Any presumptively positive samples during Screening necessitate secondary confirmation.

Should a sample test positive during the Screening phase, additional hair is extracted from the initial sample and prepped for Extraction. In this stage, drugs are extracted from the hair at much lower concentrations than those in other methodologies (e.g., urine or oral fluid), rendering hair drug screening the most challenging methodology to execute.

Positive screening results undergo Confirmation via GC/MS, GC/MS/MS, or LC/MS/MS. Presumptive positive specimens are washed before confirmation as required. The comprehensive laboratory process, from Accessioning to Confirmation, is scrutinized under both the CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
