A sample measuring 1.5 inches consisting of approximately 200 hair strands (about the thickness of a #2 pencil) closest to the scalp will yield 100mg of hair, the optimal amount for screening and confirmation processes. For EtG, add-ons, and/or tests exceeding 10 panels, 150mg of the sample is advised. It's recommended to weigh the sample on a jeweler’s scale. If scalp hair cannot be acquired, a similar amount of body hair might be used. When head hair is mentioned, it refers specifically to scalp hair. Body hair includes all other types (facial, axillary, etc.).

Process Overview

The process of laboratory analysis of a drug test result involves four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning covers the initial processing of a specimen into the lab’s system. This step includes confirming that the sample was properly sealed and shipped, assigning a random LAN (Laboratory Accessioning Number), and completing any additional data entry not provided by an electronic chain-of-custody system.

Screening consists of a quick preliminary check for drugs of abuse. Screening is a cost-effective method to exclude drug use for most samples, but for a positive screen to be court-admissible, confirmation is required. Any samples that are presumptively positive in Screening necessitate a further confirmation step.

If a sample tests presumptively positive in the Screening stage, additional hair is extracted from the original sample for preparation for Extraction. During this phase, drugs are extracted from hair at a much reduced concentration compared to other methods (e.g., urine or oral fluid), which makes hair drug screening the most challenging methodology to perform.

Confirmation of positive screening outcomes is executed using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples are cleansed prior to confirmation as necessary. The comprehensive laboratory process ranging from Accessioning to Confirmation is scrutinized under both the CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
