A 1.5-inch segment of approximately 200 hairs (equivalent to the thickness of a #2 pencil) nearest to the scalp will result in 100mg of hair, which is the optimal amount for both screening and confirmation tests. For EtG inclusions and/or tests exceeding 10 panels, a sample weighing 150mg is advisable. Utilizing a jeweler's scale to verify the specimen's weight is recommended. If head hair is not an option, an equivalent quantity of body hair can be used. References to head hair specifically denote scalp hair, while body hair pertains to all other varieties such as facial or axillary hair.

Process Overview

The key phases in the lab processing of a drug test result include Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the preliminary entry of a sample into a laboratory's framework. This phase involves confirming the sample is properly sealed and dispatched, assigning a random LAN (Laboratory Accessioning Number), and completing any additional data entry absent from an electronic chain of custody mechanism.

Screening is a preliminary rapid assessment for substances of abuse. This process is a cost-efficient strategy for discounting drug usage on the majority of samples; however, a positive screening result necessitates confirmation to ensure legal admissibility. Any samples exhibiting presumptive positivity in Screening require subsequent confirmation.

When a sample shows presumptive positivity at the Screening stage, additional hair is extracted from the initial sample and readied for Extraction. In this phase, drugs are isolated from hair at concentrations considerably lower than those in alternative methodologies (e.g., urine or oral fluid). This complexity makes hair drug screening the most challenging technique to execute.

Confirmation of any positive screening result is performed through GC/MS, GC/MS/MS, or LC/MS/MS. All samples showing presumptive positivity are cleansed before confirmation as necessary. The entire laboratory protocol from Accessioning to Confirmation is reviewed according to both the CAP (College of American Pathologists) Hair criteria and ISO / IEC 17025 accreditation standards.

The impact of hair treatments on the oxidation/decomposition of drugs in hair was also explored, as the long-term fate of drugs incorporated in the keratin matrix. Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
