Hair follicle tests benefit employers with strict drug-free policies, courts, and probation officers due to their 90-day detection range, observation in collection, and resistance to tampering. However, for detecting recent drug use up to 5 days, urine tests remain more effective.

What it is, How it works

Hair analysis is recognized as a robust method for identifying the usage of drugs and alcohol. Hair captures a lengthy history of substance use by embedding biomarkers within the fibers of the growing hair strand. When obtained near the scalp, hair can offer up to roughly a 3-month window for detecting alcohol and drugs. Hair is straightforward to gather, challenging to tamper with, and straightforward to dispatch.

A 1.5-inch section of about 200 hairs (approximately the thickness of a #2 pencil) nearest to the scalp will provide 100mg of hair, the optimal sample for analysis and verification. For EtG, additional tests, or tests exceeding 10 panels, 150mg of the sample is advised. We suggest weighing the sample using a jeweler's scale. If scalp hair isn't available, an equivalent amount of body hair may be used. When describing head hair, it specifically means scalp hair. Body hair covers all other types, such as facial and axillary.

Process Overview

The primary stages involved in the lab processing of a drug test outcome include Accessioning, Screening, Extraction, and Confirmation.

Accessioning consists of the initial handling of a sample into a laboratory's workflow. This task includes verifying that the sample was secured and transported correctly, assigning a random LAN (Laboratory Accessioning Number), and completing any supplementary data input not covered by an electronic chain of custody system.

Screening involves a swift preliminary check for drug use. Although Screening is a cost-effective technique to eliminate drug use among most samples, a positive screen must be confirmed for it to be acceptable in court. Any samples that return a presumptive positive in Screening require subsequent confirmation.

If a sample shows presumptive positivity during the Screening phase, additional hair is extracted from the original sample and processed for Extraction. During this phase, drugs are extracted from hair at much lower concentrations than other methods (such as urine or saliva), which makes hair drug screening the most challenging technique to execute.

Validation of any positive screening result is performed using GC/MS, GC/MS/MS, or LC/MS/MS. All samples suggesting positivity are cleansed before confirmation as necessary. The entire laboratory procedure, from Accessioning to Confirmation, is evaluated per the CAP (College of American Pathologists) Hair designation and the compliance with ISO / IEC 17025 standards.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
