A sample length of 1.5 inches comprising roughly 200 hair strands (comparable to a #2 pencil's diameter) nearest to the scalp yields 100mg of hair, which is the optimal sample size for both screening and definitive analysis. For EtG, additional panels, and/or tests exceeding 10 panels, a 150mg specimen is advised. We suggest utilizing a jeweler's scale for specimen measurement. In the absence of scalp hair, an equivalent amount of body hair may be used. Head hair pertains exclusively to scalp hair. Body hair includes other hair types (facial, axillary, etc.).

Process Overview

The primary stages in the laboratory analysis of a drug test outcome include Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves the initial integration of a sample into a laboratory's framework. This stage confirms proper sample sealing and shipment, assigns a random LAN (Laboratory Accessioning Number), and addresses any data entry not covered by an electronic chain of custody.

Screening entails an initial rapid assessment for substances of misuse. While Screening is a cost-efficient method to negate drug presence in most samples, a positive outcome necessitates confirmation for legal admissibility. Presumptive positives in Screening require secondary verification.

Upon a presumptive positive detection in Screening, additional hair is sourced from the original sample for Extraction. Here, substances are extracted from hair at notably lower concentrations compared to other methods (e.g., urine or oral fluid), highlighting the complexity of hair drug screening.

Confirmation of positive screening findings is performed through GC/MS, GC/MS/MS, or LC/MS/MS. Presumptive positive samples are cleansed before confirmation as needed. The comprehensive lab procedure from Accessioning to Confirmation undergoes evaluation per both the CAP (College of American Pathologists) Hair criteria and ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
