A sample measuring 1.5 inches with around 200 strands of hair (akin to the thickness of a #2 pencil) near the scalp will yield 100mg, which is optimal for testing and verification. For EtG, additional tests, or panels exceeding 10, a 150mg sample is required. It's best to measure the hair sample with a jeweler’s scale. If scalp hair is not available, a comparable amount of body hair should be used. When we mention head hair, it only pertains to scalp hair. Body hair encompasses all other types (like facial or axillary hair).

Process Overview

The laboratory processes a drug test result through four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning refers to the initial handling of a sample in a lab's system. This encompasses confirming the sample's appropriate sealing and dispatch, allocating a random LAN (Laboratory Accessioning Number), and completing any extra data entry missed by an electronic chain of custody system.

Screening is the preliminary examination for drug presence. Although it is a practical way to identify drug usage in most samples, positive screens need further validation for legal admissibility. Any samples that test positive in Screening must undergo secondary confirmation.

If a sample tests positive in Screening, additional hair is taken from the original sample for Extraction. During this phase, drugs are extracted from hair in much lower concentrations compared to other methods (such as urine or oral fluid), thus making hair drug testing the most challenging to conduct.

Confirmation for any positive screening result is achieved through GC/MS, GC/MS/MS or LC/MS/MS. Presumptive positive samples undergo washing before confirmation as necessary. From Accessioning through to Confirmation, the entire lab process is evaluated under both the CAP (College of American Pathologists) Hair classification and compliance with ISO / IEC 17025 standards.

The pitfalls encountered when the exogenous/endogenous nature of a substance is questioned were presented and discussed. Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
