What it is, How it works

Hair testing has gained recognition as an effective method for identifying drug and alcohol use. Hair strands capture biomarkers, retaining a prolonged record of alcohol and other drugs in their fibers. When hair is obtained near the scalp, it can offer a detection span of up to roughly three months for alcohol and other substances. For EtG, additional tests, and/or assessments exceeding 10 panels, a specimen mass of 150mg is advisable. Using a jeweler’s scale is recommended for weighing the sample. If scalp hair cannot be sourced, a proportional quantity of body hair is acceptable. When we refer to head hair, we specifically mean scalp hair. Body hair encompasses all additional hair types (such as facial, axillary, etc.).

Process Overview

The four fundamental steps in the lab processing of a drug test result include Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves the preliminary processing of a sample into a laboratory’s system. This process includes ensuring the sample was appropriately sealed and dispatched, assigning a random LAN (Laboratory Accessioning Number), and completing any additional data entry not covered by an electronic chain of custody system.

Screening entails an initial rapid assessment for substances of abuse. While Screening offers a cost-effective means to eliminate drug use on most samples, any positive indication must be confirmed for it to be legally valid. Samples that are tentatively positive in Screening necessitate further confirmation.

Should a sample indicate a presumptive positive during the Screening stage, additional hair is culled from the initial sample and set up for Extraction. During this stage, drugs are isolated from hair at a much lower concentration than in other testing methods (such as urine or oral fluid), making hair drug screening the most challenging testing method to execute.

Confirmation of any positive screening outcome is conducted via GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples are washed before confirmation when needed. The complete lab process from Accessioning to Confirmation is assessed under the CAP (College of American Pathologists) Hair designation and accredited to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
