If scalp hair cannot be collected, an equivalent amount of body hair might be used. By head hair, we mean just scalp hair. Body hair includes all other hair categories (such as facial, axillary, etc.).

Process Overview

The key four phases in the laboratory handling of a drug test result are Accessioning, Screening, Extraction, and Confirmation.

Accessioning constitutes the primary processing of a sample into a laboratory’s records. It involves verifying proper sealing and shipping of the sample, assigning a unique LAN (Laboratory Accessioning Number), and accomplishing any leftover data entry not covered by an electronic chain of custody procedure.

Screening consists of an initial check for substances of abuse. Although Screening is a budget-friendly means to exclude drug usage in most samples, a positive screen requires confirmation to stand up in court. Any samples preliminarily positive in Screening do require an additional confirmation.

When a sample is positive in the Screening stage, additional hair is taken from the initial collection and readied for Extraction. In this procedure, drugs are extracted from hair at significantly lower concentrations compared to other techniques (such as urine or oral fluid), making hair drug screening the most challenging technique to execute.

Confirmation of any positive screen outcome is accomplished through GC/MS, GC/MS/MS, or LC/MS/MS. All preliminary positive samples are cleaned before confirmation as necessary. The entire lab process from Accessioning to Confirmation is assessed under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 benchmarks.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
