For EtG, additional analyses, and/or tests beyond 10 panels, a 150mg sample is advised. It is advisable to weigh the specimen using a jeweler’s scale. If scalp hair is not available, an equivalent quantity of body hair may be used. By "head hair," we specifically mean scalp hair. "Body hair" encompasses all other types of hair (e.g., facial, axillary).

Process Overview

The four principal stages in the laboratory processing of a drug test are Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the initial processing of a sample into a laboratory’s framework. This step involves ensuring the sample was properly sealed and shipped, assigning a unique LAN (Laboratory Accessioning Number), and completing any necessary data entry that is absent from an electronic chain of custody system.

Screening serves as an initial speedy assessment for drugs of abuse. Although Screening provides a cost-efficient method for excluding drug use in most cases, a positive result necessitates confirmation to be valid for legal purposes. Any samples that yield a presumptive positive in Screening require a follow-up confirmation.

When a sample is presumptively positive during Screening, additional hair is extracted from the original specimen for further Preparation. During Extraction, drugs are isolated from hair at a substantially lower concentration relative to other methods (such as urine or oral fluid), making hair drug testing among the most complex methodologies to execute.

Confirmation of a positive screen is conducted using GC/MS, GC/MS/MS, or LC/MS/MS. All samples that are presumptively positive undergo washing before confirmation as necessary. The entire lab process from Accessioning to Confirmation is conducted under both the CAP (College of American Pathologists) Hair guidelines and accreditation to ISO/IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
