If scalp hair is not present, an equivalent amount of body hair can be used. When discussing head hair, we mean scalp hair exclusively. Body hair includes any other type (facial, axillary, etc.).

Process Overview

The core steps in handling a drug test in a lab are Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves the primary processing of a sample into a lab’s system. It checks if the sample was properly sealed and sent, assigns a random LAN (Laboratory Accessioning Number), and finishes any missing data entry not from an electronic chain of custody system.

Screening performs an initial rapid check for illegal drugs. While Screening is economical for excluding drug use in most samples, a positive result requires confirmation to be valid in court. Samples with a presumed positive result in Screening need further confirmation.

If a sample tests presumptively positive during Screening, extra hair from the original sample is used for Extraction. During this stage, drugs are pulled from hair in a lesser concentration than other methods (e.g., urine or oral fluid), making hair drug screening a tough technique to execute.

Verification of any positive screening result uses GC/MS, GC/MS/MS, or LC/MS/MS. Samples with assumed positives are cleaned before confirmation if needed. The whole lab procedure from Accessioning to Confirmation is inspected under both the CAP (College of American Pathologists) Hair label and the compliance with ISO / IEC 17025 criteria.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field.
