For EtG, additional testing, and/or tests beyond 10 panels, 150mg of the specimen is suggested. Use a jeweler’s scale for weighing the specimen. If scalp hair isn't available, a similar quantity of body hair may be utilized. "Head hair" specifically means scalp hair only. "Body hair" pertains to any other hair types (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result includes four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial sample processing phase into a laboratory's framework. This phase checks that the sample was sealed and shipped correctly, assigns it a random LAN (Laboratory Accessioning Number), and concludes any extra data entry not covered by an electronic chain of custody system.

Screening offers a preliminary rapid assessment for drugs of abuse. Although it's a cost-effective method to rule out drug use in most samples, a positive result in Screening needs confirmation to be valid in legal contexts. All samples that show a presumptive positive in Screening necessitate secondary confirmation.

If Screening results are presumptively positive, more hair is extracted from the initial sample to prepare for Extraction. During this stage, drugs are drawn from hair in much lower concentrations than in other methods (e.g., urine or oral fluid), which is why hair drug testing is notably challenging.

Confirmation of any positive Screening outcome is done through GC/MS, GC/MS/MS, or LC/MS/MS. Presumptive positive samples are cleaned before necessary confirmation. The entire lab process from Accessioning to Confirmation is reviewed following both the CAP (College of American Pathologists) Hair guidelines and ISO / IEC 17025 accreditation standards.
