For EtG, additional tests, and/or panels exceeding 10, a specimen of 150mg is advised. Using a jeweler’s scale to weigh the specimen is suggested. If scalp hair can't be obtained, the equivalent quantity of body hair is acceptable. When discussing head hair, it pertains solely to the scalp. Body hair encompasses all other varieties (facial, axillary, etc.).

Process Overview

The four key phases in processing a drug test in the lab are Accessioning, Screening, Extraction, and Confirmation.

Accessioning encompasses the preliminary handling of a sample within the lab’s system. This phase checks that the sample was securely sealed and shipped, assigns a random LAN (Laboratory Accessioning Number), and completes any outstanding data entry not covered by an electronic chain of custody.

Screening serves as a preliminary assessment for substance abuse. Although it provides a cost-effective means of ruling out drug presence in most samples, a positive screen must be verified for legal admissibility. Samples testing presumptively positive require secondary confirmation.

Should a sample be presumptively positive in Screening, additional hair is extracted from the original specimen for preparation in Extraction. In this phase, drugs are retrieved from hair at significantly lower concentrations than other methods (e.g., urine or oral fluid), establishing hair drug testing as a challenging practice.

Confirmation follows for any positive screen results using GC/MS, GC/MS/MS, or LC/MS/MS. Necessary presumptive positive samples undergo washing before confirmation. From Accessioning to Confirmation, the lab process is reviewed under the CAP (College of American Pathologists) Hair designation in line with ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
