What it is, How it works

Hair captures a long-term record of alcohol and drug usage by trapping biomarkers within the fibers of the growing hair strand. When sampled close to the scalp, hair can provide a detection window of up to roughly three months for alcohol and various drugs. It is easy to collect, challenging to tamper with, and convenient to transport.

A 1.5-inch segment consisting of approximately 200 hair strands (about the thickness of a #2 pencil) nearest the scalp will yield 100mg of hair, which is the optimal sample size for screening and confirmation. For EtG, additional requirements, and/or tests exceeding 10 panels, 150mg of the sample is advisable. The use of a jeweler’s scale is suggested for weighing the specimen. If scalp hair is not present, an equivalent amount of body hair can be gathered. References to head hair pertain solely to scalp hair. Body hair encompasses all other hair types (facial, axillary, etc.).

Process Overview

The laboratory process for a drug test outcome involves four primary stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning consists of the initial integration of a sample into a laboratory’s system. The process entails confirming proper sealing and shipping of the sample, assigning a random LAN (Laboratory Accessioning Number), and completing any additional information not already provided by an electronic chain of custody system.

Screening performs an initial rapid assessment for drugs of abuse. Although Screening is an economical method to exclude drug use in most samples, a positive screen must be confirmed to be considered valid in court. All samples that are presumptively positive after Screening require further confirmation.

When a sample is presumptively positive at the Screening phase, additional hair is extracted from the original specimen and prepared for Extraction. This stage involves isolating drugs from hair at significantly lower concentrations than other methods (e.g., urine or oral fluid), making hair drug screening a particularly challenging methodology to execute.

Confirmation of any positive result from Screening is executed via GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples are cleansed prior to confirmation as necessary. The comprehensive laboratory procedure from Accessioning to Confirmation is assessed under both the CAP (College of American Pathologists) Hair designation and compliance with ISO / IEC 17025 standards.
