What it is, How it works

The collection process is straightforward, it's challenging to tamper with, and it's easy to transport.

A sample measuring 1.5 inches long and containing approximately 200 strands of hair (equivalent to the thickness of a #2 pencil) close to the scalp will yield 100mg of hair, which is the optimal amount for screening and verification. For EtG, additional substances, and/or tests exceeding 10 panels, 150mg of the sample is advisable. We suggest measuring the sample using a jeweler’s scale. If scalp hair cannot be obtained, a comparable amount of body hair can be used. When we mention head hair, we refer solely to scalp hair. Body hair includes all other types of hair (such as facial and axillary).

Process Overview

The laboratory processing of a drug test result consists of four key steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves the initial incorporation of a sample into a lab's system. This process ensures the sample was properly sealed and transported, assigns a random LAN (Laboratory Accessioning Number), and completes any extra data input not included by an electronic chain of custody system.

Screening involves a preliminary quick assessment for substances of abuse. Although Screening is a budget-friendly method to rule out drug usage in most samples, a positive result necessitates confirmation for court admissibility. Samples that are presumptively positive during Screening require an additional confirmation process.

If a sample registers as presumptively positive during the Screening phase, more hair is extracted from the original sample and prepped for Extraction. In this phase, drugs are extracted from hair at a significantly lower concentration compared to other techniques (e.g., urine or oral fluid), making hair drug screening the most challenging technique to execute.

Confirmation of any positive screening outcome is performed via GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples undergo washing before confirmation, as necessary. The complete lab process from Accessioning to Confirmation is reviewed under both the CAP (College of American Pathologists) Hair certification and ISO / IEC 17025 standards accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
