What it is, How it works

Hair testing has gained recognition as a robust method for identifying drug and alcohol usage. Hair serves as a long-term record of drug and alcohol consumption by embedding biomarkers in the strands of the hair as it grows. When hair is collected near the scalp, it can offer a detection period of up to about 90 days for drugs and alcohol. Hair collection is straightforward, somewhat challenging to tamper with, and easy to transport.

A 1.5-inch portion consisting of approximately 200 hair strands (roughly the width of a #2 pencil) near the scalp provides 100mg of hair, which is the perfect sample size for both screening and confirmation. For EtG, add-ons, and/or tests beyond 10 panels, 150mg of hair is advised. Using a jeweler’s scale to weigh the specimen is recommended. If scalp hair is not available, a comparable quantity of body hair can be obtained. Reference to head hair specifically indicates scalp hair. Body hair encompasses all other hair varieties (like facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result involves four principal steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves the preliminary processing of a sample into the laboratory’s system. This step includes confirming that the sample was sealed and shipped correctly, assigning a random LAN (Laboratory Accessioning Number), and completing any additional data input not covered by an electronic chain of custody system.

Screening consists of an initial rapid test for drugs of abuse. While Screening is a cost-efficient approach to eliminate drug usage for the majority of samples, confirming a positive screen is necessary for it to be legally valid. All samples that test presumptively positive during Screening must undergo a secondary confirmation.

If a sample tests presumptively positive during the Screening stage, additional hair is extracted from the original specimen and processed for Extraction. In this phase, drugs are isolated from hair at a significantly lower concentration compared to other methodologies (e.g., urine or oral fluid), which makes hair drug testing the most complex method to perform.

Confirmation of any positive screening result is achieved using GC/MS, GC/MS/MS or LC/MS/MS. All presumptive positive samples undergo washing before confirmation as needed. The complete laboratory procedure from Accessioning to Confirmation is reviewed under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.

Clinical Applications of hair testing are witnessed by the insight on environmental tobacco smoke exposure, assessed by hair analysis in a population of students and the study of in utero-exposure to drugs of abuse through neonatal hair analysis. Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
