For tests involving EtG, additional panels, or tests surpassing 10 panels, a 150mg sample is suggested. We advise using a jeweler’s scale to weigh the sample precisely. In the absence of scalp hair, an equivalent volume of body hair may be gathered. When discussing head hair, it specifically refers to scalp hair, while body hair encompasses all other types of hair (facial, axillary, etc.).

Process Overview

The primary stages in analyzing a drug test result at the laboratory include Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the preliminary handling of a sample entering the lab’s system. This process verifies proper sealing and shipping of the sample, assigns a unique LAN (Laboratory Accessioning Number), and completes any extra data entries not captured by an electronic chain of custody system.

Screening includes an initial rapid assessment for abused substances. Although Screening is a cost-efficient method to rule out drug use in most samples, a positive screen must be confirmed for legal validity. Any sample flagged as presumptive positive during Screening mandates further confirmation.

When a sample is flagged as presumptively positive during Screening, additional hair from the original sample is utilized for Extraction. Here, drugs are isolated from the hair at significantly lower concentrations compared to other methods (such as urine or oral fluid). This makes hair drug screening one of the most complex methodologies to execute.

The Confirmation of any positive result from Screening is carried out using GC/MS, GC/MS/MS, or LC/MS/MS techniques. All samples marked as presumptive positive are cleaned before Confirmation as required. The entirety of the lab procedure, from Accessioning to Confirmation, is scrutinized under both the CAP (College of American Pathologists) Hair classification and the ISO / IEC 17025 standards accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
