What it is, How it works

Hair collection is straightforward, less prone to tampering, and convenient for shipping.

A 1.5-inch hair sample, consisting of roughly 200 strands from near the scalp (approximately the thickness of a #2 pencil), results in 100mg of hair, which is ideal for both screening and confirmation. For EtG, additional testing, or panel tests exceeding 10, a 150mg sample is advised. Using a jeweler’s scale to weigh the sample is recommended. When scalp hair is not available, an equivalent quantity of body hair is an alternative. References to head hair indicate scalp hair solely, whereas body hair pertains to other types like facial or axillary hair.

Process Overview

The laboratory processing of a drug test result consists of four major parts: Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves the initial incorporation of a sample into the lab system. This step ensures proper sealing and shipping of the sample, assigns a random LAN (Laboratory Accessioning Number), and includes any extra data entry not covered by an electronic chain of custody system.

Screening is an initial, swift evaluation for drugs. Although Screening efficiently rules out drug usage in most samples, a positive screen must be confirmed for legal validity. Samples with a presumptive positive in Screening require further confirmation.

Presumptive positive results in Screening lead to additional hair being drawn from the initial specimen for the Extraction process. In Extraction, drugs are removed from hair at much lower concentrations than in other techniques (e.g., urine or oral fluid), making hair drug screening the most challenging to perform.

Confirmation of any positive findings is achieved using GC/MS, GC/MS/MS, or LC/MS/MS. Prior to confirmation, all samples presumed positive undergo washing as necessary. The entire lab process, from Accessioning to Confirmation, is evaluated under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
