When harvested near the scalp, hair can offer a detection period of up to roughly 3 months for alcohol and various drugs. It is straightforward to collect hair, relatively hard to adulterate, and convenient to transport.

Obtaining a 1.5-inch sample of around 200 hair strands (approximately equivalent to the thickness of a #2 pencil) nearest to the scalp will yield 100mg of hair, which is the optimal amount for testing and verification. For EtG, additional screenings, and/or testing exceeding 10 panels, a 150mg specimen is advisable. We advise measuring the specimen using a jeweler’s scale. If scalp hair isn't accessible, an equivalent quantity of body hair can be used. By mentioning head hair, we reference scalp hair exclusively. Body hair signifies other hair types (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result includes four key phases: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the preliminary handling of a sample within a lab’s system. This process involves confirming that the sample was sealed and shipped appropriately, allotting a random LAN (Laboratory Accessioning Number), and finalizing any extra data input not incorporated by an automated chain of custody system.

Screening consists of an initial rapid assessment for substance abuse. Though Screening is a budget-friendly means to dismiss drug usage for most samples, a positive result must be validated to be acceptable in legal proceedings. Any samples preliminarily positive in Screening demand a secondary verification.

Should a sample be tentatively positive in the Screening phase, further hair is extracted from the original specimen and readied for Extraction. During this phase, drugs are isolated from hair at significantly lower amounts than other procedures (such as urine or oral fluid), hence hair drug screening is the most challenging methodology to execute.

Verification of any positive screening result is performed through GC/MS, GC/MS/MS, or LC/MS/MS. All tentatively positive samples undergo washing before confirmation if necessary. The comprehensive laboratory process from Accessioning to Confirmation is overseen under both the CAP (College of American Pathologists) Hair classification and accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
